Care Following Hospital Discharge

Coming home after a spell in hospital can sometimes cause anxiety - both for the patient and their family and friends. At times like this, additional support can go a long way to help build confidence, improve mobility or encourage greater independence following a period of constant care and attention.

At Direct Health, our experienced Care and Support Workers can give you that additional support to help you settle back in at home. We can provide practical assistance which might include help with shopping, cleaning, laundry or food preparation. Or we can help with personal care including washing, dressing or assistance with medication.

Our service can be called upon at short notice to enable you to return home from hospital as soon as possible and we offer a comprehensive range of support services that are tailored to meet your particular needs and wishes.

We recognise that everyone’s needs are different. You may have not have family close by. Or they may have commitments with work and family that means that they cannot be available every day. We can work with you to identify the support needed and to complement the care and support that your own family may be able to give.

We can give you as much support as you need, for as long as you need it. It’s your choice and you’re always in control. We work hard to ensure your return home is a successful one and to enable you to remain independent in your own home.
