Staywell Service
People aged 65 or over living across the borough of Rochdale, who feel that they could benefit from up to six weeks of support from a Staywell Worker to build confidence around the house, socialise and get involved with community activities, or accessing services that improve health and wellbeing, can contact our Staywell Coordinator on staywell@direct-health.co.uk or 01706 360008.
Many older and frail people live alone and may require support other than health/social care. The support Direct Health are providing includes a minimum of weekly ‘safe and well’ meetings, medication checks, appointment checks, home risk assessments, problem-solving/solution finding, support direction into social activities. This service provides social care - there is no personal care involved.
Direct Health is keen to build relationships and develop links with relevant health and social care agencies, secondary care services and voluntary sector services to optimise the care and support received by frail older people.
